Registration Terms and Conditions

Please review the Terms and Conditions stated below.  By registering yourself or your child to participate at Manchester Athletic Association you are acknowledging that you have read, understood, and accept the Terms and Conditions of participation.

Power of Attorney for Consent to Medical Care: 
I/We, the parents or legal guardian of the Player named above, hereby authorize the bearer of this document, or a copy of same, to consent to any kind of medical care that he or she may deem necessary or appropriate, including, but not limited to, treatment for fractures, cuts, abrasions, surgery, blood transfusions and the administration of drugs.  In order to assure that the Player receives prompt medical care and treatment when necessary, I/we hereby release any and all health care providers who provide said services to the Player in reliance on this Power of Attorney from any and all claims, suits, or liabilities arising out of or with respect to said care and treatment.  This Power of Attorney is dated below and shall be valid for one year from said date.

 

Reliease and Waiver: 
The undersigned, being the parent or legal guardian of the Player named above, as inducement for and in consideration of the agreement by Manchester Athletic Association (MAA) that the Player may take part in athletic activities organized or sanctioned by MAA, hereby agrees and covenants that MAA and its officers, directors, employees and agents shall not be liable in any way for any personal injuries, including death, or damage to property that the Player may sustain or cause in any manner which results from or arises out of the Player’s preparation for or participation in such activities.  The undersigned hereby fully and forever releases and discharges and covenants not to sue MAA and its officers, directors, employees and agents from any and all claims, demands, damages, rights of action or causes of action, present or future, whether known or unknown, anticipated or unanticipated, resulting from or arising out of the Player’s preparation for or participation in such activities.  The undersigned acknowledges and assumes full responsibility for, and risk of, such personal injury, including death, and damage to property and for all medical and other expenses incurred as a result of the Player’s activities.  Moreover, the undersigned agrees that prior to participation in or preparation for such activities, he/she will inspect the facilities and equipment to be used, and if he/she believes anything is unsafe, he/she shall immediately advise their manager, coach or supervisor of such condition(s) and refuse to participate in or prepare for such activities.  This Release and Waiver shall be binding on the heirs, executors, administrators and assigns of the undersigned and the Player.  The undersigned hereby certifies that he/she has read and fully understands this Release and Waiver.

 

Important Notice:  Release and Waiver: 
The undersigned, being the parent or legal guardian of the Player named above, has been informed through this notice that the Manchester Athletic Association (MAA) does not perform background checks on the managers, coaches or volunteers who may interact with their child.  It is the responsibility of each parent/guardian to take whatever measures they feel necessary with their child’s respective managers, coaches and volunteers to insure the safety of their child while participating in any event directly or indirectly associated with MAA.  By accepting below the undersigned understand and agree to hold MAA and its directors, officers, employees and agents harmless from any claims whatsoever that could arise from sexual misconduct involving their child.

 

PAYMENT AGREEMENT: 
Two concession duties are required for each MAA family.  I understand that if I do not show up for my assigned concession work duty, my child is immediately suspended from the program until the association treasurer receives the appropriate fine. Fines ARE $200.00 for a missed Concession/Work Duty. Please also note that work duty assignments cannot be rescheduled by any member of the association's board of directors. If you cannot work your assigned duty, you may trade with another family or pay the fine in advance. The undersigned hereby agrees to have any fines which they may incur automatically charged to the credit card they used for registration and authorizes Manchester Athletic Association to charge any such fines to said credit card.  Please refer to the FAQ Web Page on this site for more information on work assignments.
